Many serious accidents have occurred due to a machine being started when it was supposed to be completely shut off to prevent maintenance or repair. These accidents are often caused by stored energy that isn't properly released. Lock-out tag-out (LOTO) systems can prevent these accidents by making sure that machines are off until the person who is authorized releases them.
The LOTO system is comprised of a series of locks tags, locks, and other equipment that are used to secure, isolate or block the machine from electrical sources. All "affected", "authorized", and "qualified" personnel should be instructed on the use of lockout and tagout devices. These devices include but not restricted to chains, locks padlocks, wedges adapter pins and self-locking fasteners. Each device is identified and labeled, indicating the date and time at which it was used along with the name and title of the person authorized, and the purpose of the lockout or tagout (repairs, maintenance and maintenance, etc.). It should be clear that these devices can't be removed until a thorough LOTO procedure has been followed.
